What can I do if I have issues with a survey?

Each survey is assigned a unique three word phrase that is presented prior to beginning a survey called a Survey Code. Please take note of this phrase as it will help to identify the survey in your survey-taking history. 

If an error occurs while taking a survey, you can report the issue by clicking the Report Issue button located on the My Points page. Be sure to select the survey in which you encountered the issue and explain the problem in as much detail as possible. Attaching a screenshot of the error screen will assist us in identifying the survey issue more efficiently, allowing us to respond in a more timely, thorough manner.
